Implementación de un Programa de Seguimiento Farmacoterapéutico en Pacientes Adultos Mayores con Hipertensión Arterial del Centro de Atención Ambulatorio del IESS de Santo Domingo de los Colorados

Abstract

Implementation of a Pharmacotherapeutic Follow-up Program in Elderly Patients with Arterial Hypertension of the Outpatient Care Center of the Ecuadorian Institute of Social Security (IESS) of Santo Domingo de los Colorados, since there was no control in this group of patients within the institution, being this a justification for the implementation of this program. It was developed from October 2010-February 2011, using inductive-deductive, individualized and intensive method to 30 patients with field technique a prospective follow-up of pharmacological therapy, dader, training. Of the 26 patients who participated in the interventions, 27 DRP (Medication-Related Problems) were found, of which 22 were solved. The most frequent was DRP 4 and 1, then DRP 2, 5,3 and finally DRP 1. The application of this program contributed to the patient's knowledge of their pharmacological therapies, which was reflected in adherence to treatment and corroborated by the values achieved at the end of the interventions. Emphasize that patients should take their medication correctly, and that patients should be informed of the possible side effects of the drugs and that, despite this, solutions should be sought to ensure that patients continue with their antihypertensive treatment. It is recommended to continue educating the elderly population to help in the early detection of this health problem.
