Propuesta para la implementación del sistema de dispensación de medicamentos por dosis unitaria en el servicio de cirugía del Hospital Provincial Alfredo Noboa Montenegro, Guaranda

Abstract

The objective of this degree work was to develop a proposal for the implementation of the Unit-Dose Drug Distribution System (SDMDU), applicable in the Surgery service of the Alfredo Noboa Montenegro Provincial Hospital, Guaranda. The methodology used was non-experimental, descriptive-observational and quantitative, during the January - March 2019 period. The research was carried out in three phases; In the first, a checklist was used, which determined that 17% complied, while 83% did not comply in the medication distribution process, prescription errors were 85.6%, for which, 390 medical prescriptions were reviewed, in addition, it was determined that the re-labelling and return of medicines were not performed, in the second phase the Standard Operating Procedures (SOPs) were used, with which the pharmaceutical intervention was conducted, therefore the prescription errors decreased to 36.3%, 151 pharmacotherapeutic profiles were prepared, 125 oral solid medications were re-labelled, the medication was also prepared for each hospitalized patient, and 104 medications were re-entered to the pharmacy at a cost of US$ 81.70, with which compliance with the patient's pharmacological therapy is ensured and in the third, as actions for improvement, the SOPs for Pharmacovigilance, management of high-risk medicines, concentrated electrolytes and LASA were drawn up. It is concluded by applying the SOPs per unit dose and with the action of the pharmaceutical company, prescription errors were reduced to 49.3%, which is why the implementation of SDMDU is proposed.
