Abstract

This titling work exposes the case of a 60-year-old patient with type 2 diabetes mellitus with little adherence to treatment, who went to the doctor for presenting urinary symptomatology and a history of falling from her height days before admission. At the time of her hospitalization, the patient is with evident neurological deterioration and hemodynamic instability. Complementary examinations were performed, and it was classified as a septic shock of urinary and pulmonary focus with multi-organ failure, diabetic, and fracture of 4 coastal arches. Broad-spectrum antibiotic therapy was initiated, resuscitation with crystalloids and administration of insulin with favorable evolution, however in chest X-ray it was evidenced opacity that compromised 2/3 of the left lung field, compatible with lung abscess, so it needed to be referred to the Teaching General Hospital of Riobamba for an exploratory thoracotomy. Sepsis and septic shock were the leading causes of hemodynamic and tissue alteration of the patient, pathological conditions that could be reversed promptly, thus concluding that early diagnosis and successful treatment, through effective hemodynamic restoration together with The specific treatment for the causative agent are the bases of the procedure.
